NOTE: that you must return and complete this form in order to be eligible to win. You must submit an original file and a thumbnail image. Original files must be .doc, .pdf or .jpg files AND should not exceed 2MB. Thumbnail images should be .jpg format and should be exactly 400 pixels wide and the appropriate height. You MUST include a thumbnail even if you are hosting your image on your web site.

Your entry files MUST be available to users - either posted on your own web site or submitted to UniKeep through this form.

By submitting an entry here, you grant UniKeep LLC the right to link to your site and to display your document physically, electronically and in print as representational of customized UniKeep covers. You also grant UniKeep customers and web site visitors full rights to use and print your document for normal individual use and certify you have full ownership or authorization to use and bestow rights to use the content in your entry document. Neither UniKeep, nor its customers or visitors, will receive any rights to use your document or prints of your document for commercial or resale purposes.

Your entry eligibility is subject to the Contest Rules & Guidelines as specified.

All entries will be judged according to the month in which they are received. Entries must be received by midnight Eastern Standard Time on the last calendar day of the month to be eligible for that month's awards. All entries must be received by midnight (EST) December 31, 2008 to be eligible for grand prizes.

By submitting an entry, you also give and grant for a period of 99 years (hereinafter referred to as the "Term" to UniKeep LLC and its respective licensees, successors and assigns (herein collectively called the "licensed parties"), the right to use, publish and copyright your name and any correspondence that is written, emailed or spoken, as well as voice, picture, portrait and likeness that you may voluntarily provide for use in all media and types of advertising and promotion of UniKeep LLC, a product or service of UniKeep LLC.

You agree that no advertisement or other material need be submitted to you for any further approval and the licensed parties shall be without liability to you for any distortion or illusionary effect resulting from the publication of your picture, portrait or likeness.
